You can be granted access to another patient’s medical records if that individual provides the hospital with written permission and if applicant is 18 years or older. As a 'patient proxy,' you will have access to the individual's personal health information. Authorization for proxy access to patient’s account is valid until revoked by the patient.
For access to a child’s record, the parent or legal guardian can either contact the Doylestown Hospital Patient Portal office and request access, or fill out and sign the authorization form. The parent or legal guardian can also designate access to the child’s record to another person by filling out and signing the authorization form. Authorization for proxy access to a child’s record will automatically expire at the age of 18, and require the adult child to re-issue authorization with their own signature.
The Patient Portal Office is open Monday - Friday, from 8:30am - 5:00pm.
